The terms for using this site and buying from its store.
Who you are dealing with
This store belongs to Balencia. Balencia's studio is at 8010 E McDowell Rd, Suite 209, Scottsdale, AZ 85257. Platform Health runs the shop, cart, checkout and accounts for it and is the merchant of record: when you buy from the shop you pay Platform Health, which sells on Balencia's behalf, and Stripe processes the payment. By using the store you agree to these terms and to the Privacy Policy.
What you are buying
Health products, medical foods, supplements and programs, which are not medical treatment, a diagnosis or a prescription. The store also lists prescription products, including compounded medications. Nothing in the store recommends a product for you.
Prescription products
Any prescription product, including any compounded medication, is prescribed solely by an independent licensed clinical provider after an online visit, and only if that provider decides it is appropriate for you. Balencia does not decide whether you receive it. A prescription product is bought from the independent provider's service, not from this store: the online visit, the checkout and its terms are theirs, and a licensed pharmacy dispenses and ships what they prescribe. Compounded medications are not approved by the FDA, and the FDA does not review them for safety, effectiveness or quality before they are marketed. A compounding pharmacy being licensed or regulated, including a “503A” pharmacy, does not mean the FDA has approved the medications it compounds.
Orders, shipping and returns
The shop ships within the United States. Balencia and the supplier that ships your order are responsible for fulfilment, returns and product warranties. The store is not a substitute for in-person care and does not handle medical emergencies; for an urgent concern, call 911.
Who is paid
Balencia, and Platform Health, which runs this shop for it, earn money when you buy from the shop. When you start an online visit for a prescription product, the independent provider's service may also pay Platform Health a referral fee. The independent provider decides on their own whether to prescribe, and whether you buy anything here is up to you.
Subscriptions
Subscription items renew at the interval and price shown on the product page and in your cart, before you check out. You can cancel from your account at any time, under Subscriptions. Cancelling takes effect at the end of the period you have already paid for, and we do not refund a period that has already begun.
Eligibility and use
You must be 18 or over. Please do not resell what you buy here, give false information at checkout or in your account, or scrape the store without permission.
Warranties, termination and governing law
The store and its materials are provided as is, without warranty of merchantability or fitness for a particular purpose; to the maximum extent the law permits, Platform Health and the clinics it sells for disclaim liability for indirect, incidental or consequential damages. Access may be suspended or ended for a breach of these terms or for fraud. These terms are governed by the laws of the State of Delaware, USA, without regard to conflict-of-laws principles; disputes will be resolved in the federal or state courts of New Castle County, Delaware.
