If you experienced an unexpected reaction to a medication prescribed by Platform Health — or any medication — please report it. Your report informs both our protocols and the FDA's population-level safety database.

The FDA's MedWatch program tracks medication safety signals across the population. Anyone can report — clients, family members, providers. Your submission helps the FDA identify trends a single clinic could never see.

Tell us directly

We track every adverse event from clients in our care so we can do three things:

  • Reach out and provide medical follow-up
  • Identify whether any prescribing protocol needs to change
  • Cross-report to the FDA on your behalf if appropriate
